METAL BENCHES | $10,000
LUEDER BENCHES | $2,500

  • Benches will be the current design standard of the City of Heath.
  • Benches may not be painted, stained, decorated, or altered in any way.
  • Benches are the property of the City of Heath.

Recognition:

Plaques hold 3 lines of text, consisting of approximately 155 characters including spaces and punctuation. Please keep the message for the plaque as brief as possible. Donor(s) will see a proof of the plaque and must approve the final design before it will be cast.

TRAIL ENGRAVING | $2,500

  • Engraving will be located within a 10’x20’ section of existing trail.

TREES | $1,000

  • Trees will be the current species standard of the City of Heath.

TAX DEDUCTABLE DONATION

All contributions to a municipality are tax deductible in accordance with Section 170(c) (1) of the Internal Revenue Code if made exclusively for a public purpose such as the City’s park system.

While City staff will take every consideration into placement of the bench, tree or future trail segment recognition, final decisions are at the sole discretion of the City, as per the City Donation Policy, approved on February 24, 2026.

Acknowlege
By submitting this form, I understand that the item installed by donation is the property of the City of Heath, who will care for and maintain the item. I cannot add, alter, or affix anything to the item at any time per City Ordinances. Should the bench deteriorate due to natural weathering, it is the responsibility of the City to get a replacement for the donation.

After reviewing your request, a City employee will reach out to you to regarding your location request, message, and payment will be collected by check. The check will be made out to the City of Heath. A final receipt will be provided once payment has been processed.
